I hereby authorize the director of the Bobby “BC” Collins Basketball Camp to act in any emergency requiring medical attention. My child is physically able to participate in basketball camp activities according to our family physician.

I hereby authorize the director of the Bobby “BC” Collins Basketball Camp to act in any emergency requiring medical attention. My child is physically able to participate in basketball camp activities according to our family physician.

I will not hold the coaches, instructors, or the Bobby “BC” Collins Basketball Camp responsible for any liability due to injuries result- ing from the Bobby “BC” Collins Basketball Camp. I realize any injuries resulting from this camp will be covered by my family's health insurance and/or camp insurance.
